MMA and Boxing: The Risks of Repeated Head Impacts

Boxing and mixed martial arts (MMA) are demanding sports that require fitness, discipline and technical skill. They also expose athletes to repeated impacts to the head. Unlike many other contact sports, strikes to the head are a normal part of competition. This means that brain health should be treated as an important part of training, medical care and recovery.
A concussion can occur after a punch, kick or other impact causes the brain to move rapidly inside the skull. A fighter does not need to lose consciousness to have a concussion. Possible symptoms include headaches, dizziness, confusion, balance problems, nausea, memory difficulties and sensitivity to light or noise. Symptoms may appear immediately or develop later after the fight or training session (Centers for Disease Control and Prevention [CDC], 2025a).

The Risk Goes Beyond Knockouts
Knockouts are the most visible form of head trauma in combat sports, but they are not the only concern. Fighters can receive many smaller head impacts during bouts and sparring sessions. These impacts may not produce obvious symptoms, but they still contribute to the athlete’s total exposure.
Research involving professional fighters has found an association between greater exposure to repetitive head trauma, lower brain volumes and slower processing speed. This does not mean that every boxer or MMA fighter will develop permanent brain damage. It does show that total exposure matters and that attention should not be limited to recorded knockouts or diagnosed concussions (Bernick et al., 2015).
Research on boxing sparring has also found temporary changes in brain function and motor control after repeated head impacts. The athletes in the study did not necessarily have a diagnosed concussion. This suggests that routine sparring may affect the brain even when fighters do not notice clear symptoms (Di Virgilio et al., 2019).
MMA creates a different pattern of exposure from boxing because it includes punches, kicks, elbows, takedowns and ground fighting. An analysis of thousands of professional MMA fights found that competitors could receive large numbers of head strikes during competition. Head impacts may continue until the referee stops the fight, particularly during a technical knockout (Mańka-Malara et al., 2022).
Recognising a Possible Concussion
Coaches and fighters should look for changes after a hard strike. A fighter may appear confused, move unusually or have difficulty following instructions. They may forget part of the fight or complain of headaches and blurred vision. Changes in mood, sleep and concentration can also occur.
Serious warning signs include repeated vomiting, seizures, slurred speech, increasing confusion, weakness and difficulty waking up. These signs require urgent medical care (CDC, 2025a).
A fighter with a suspected concussion should stop training or competing immediately. They should not return to sparring later on the same day. Returning too soon may increase the risk of another concussion while the brain is still recovering (CDC, 2025b).
Reducing the Risk
Combat sports cannot be made completely risk-free, but unnecessary head impacts can be reduced. One of the most practical measures is limiting hard sparring. Fighters can improve technique, timing and fitness through pad work, defensive drills, controlled sparring and non-contact exercises. Hard sparring does not need to be part of every training session.
Coaches should monitor how often athletes receive head impacts in both training and competition. A fighter who has recently competed or received several hard strikes may need additional recovery time. Training plans should consider total exposure rather than treating each session separately.
Headguards can reduce cuts and certain direct injuries, but they cannot completely prevent a concussion. The brain can still move inside the skull after an impact. Fighters should therefore not take greater risks simply because they are wearing protective equipment.
Medical suspensions and recovery periods should also be taken seriously. A fighter may feel pressure to return quickly, especially when a competition has already been scheduled. However, a return to sport should happen gradually and under the supervision of a healthcare professional. The CDC recommends a staged return that begins with normal activities and progresses through light exercise, harder training and full competition. Each stage normally takes at least 24 hours and the athlete should stop if symptoms return (CDC, 2025c).
Protecting Long-Term Brain Health
Brain health should be monitored throughout a fighter’s career. Fighters should feel able to report symptoms without being described as weak or undisciplined. Coaches, medical professionals and sports organisations all have a responsibility to create an environment in which health comes before a single fight.
Boxing and MMA will always involve physical risk. The goal is not to remove competition from the sports. The goal is to reduce avoidable damage through controlled training, honest symptom reporting and proper recovery. A strong fighter should not only know how to continue. They should also know when it is safer to stop.
